OpenGate’s graduate level internships and post grad internships are a comprehensive training program that will prepare you with the skills needed to ethically, efficiently, and profitably run your dream private practice.
Supervised billing services are available by subscription service for a monthly fee.
Clinical consultation is also available for those who want to work on private practice management without clinical supervision or supervised billing.
Clinical Skills
Develop yourself as a professional by learning to how to balance your personal style, well-selected tool kit, and strong boundaries, with the navigation of the challenging clinical aspects of this field, such as:
- Diagnosis and treatment planning
- Case conceptualization
- Ethical, effective rapport building
- Complex trauma cases and treatment resistance

Deepen your theoretical orientation, practice methods and approaches, gain confidence and mastery, and authentically connect with your clients. These clinical skills will build the competence you need to start your independent practice.


Practice Management
Outside of the sessions, there is a lot that goes into running a private practice. We will navigate through everything you need to start a small business that will run smoothly, efficiently, legally, and ethically.
Each counselor trainee will leave the program able to set up and manage their private practice as a small business in an efficient way using Simple Practice software. Some topics covered are:
- Licensing and credentialing processes
- Documentation
- Compliance
- Claims and Billing, bookkeeping, taxes
- Professional Will Writing
- Policy Writing
- Marketing, and more!
Supervised Billing
OpenGate offers a unique supervision program that combines supervised billing with hands-on training in how to run a private practice. Unlike traditional placements, our model prepares you for both the clinical work and the business side of practice so you can step into independent licensure with confidence.
Supervised billing is available to Interns, Graduate students, Master’s-level counselors, and those pursuing LCMHC, LCSW, or LADC licensure. Interns bill Vermont Medicaid, MVP, and sometimes Cigna, and post-graduate clinicians may bill additional insurers such as BCBSVT. All trainees may also accept private-pay clients.
What makes OpenGate different is that we don’t just bill for you—we teach you how to do it yourself. By the time you’re licensed, you’ll know how to manage your own claims and workflows, which means you’ll never need to hire or pay a separate biller.
This program is ideal for clinicians who want to build a full, thriving private practice and gain the skills—clinical and administrative—to run it independently.

About the Program
The OpenGate Supervision Program is a paid, graduate-level internship or postgraduate externship designed for clinicians pursuing their Vermont LCMHC, Clinical Social Worker, or LADC license. Our goal is to help you build a fully functioning, independently licensed private practice—one that reflects your clinical strengths, your values, and the kind of therapeutic life you want to create.
Training in this program provides a balanced focus on both the front-of-house (clinical practice) and the back-of-house (private practice management). You’ll learn every element needed to run a legal, ethical, efficient, and sustainable practice—including licensure and compliance, documentation, advertising, collaboration, insurance credentialing, claims and billing, bookkeeping, and systems management. Clinically, we work through issues specific to your caseload as well as those that arise in the broader development of your practice. When challenging or complex cases emerge, or when personal and professional difficulties surface, supervision is here to provide steady guidance and a safe space to think and process.
As Clinical Director, Kathleen provides most administrative training, practice management support, and supervised billing. You will also work closely with your assigned supervisor, who may be Kathleen or another member of our growing supervision team—each bringing extensive training, clinical experience, and a shared commitment to ethical, grounded, thoughtful practice. Your supervisor will help you develop your clinical voice, navigate challenges, and build confidence in both your therapeutic work and your business.
It’s important to know that OpenGate is not designed to be a side hustle. The structure, supports, and financial model of the program work best for clinicians who plan to build a full-time private practice (typically 18–28 clients per week). This program is a good fit for the serious, self-motivated entrepreneur who is ready to invest in their future and grow a meaningful, sustainable practice.

How it Works
First, envision your dream practice. Then, reach out and we’ll take it from there! Full transparency and fairness are important parts of the OpenGate business model. I put it all out here so you can make an informed decision about whether this program might be for you. Here’s the scoop, but feel free to ask about anything you wonder that isn’t laid out here.
Fee Structures:
Monthly Subscription Services for Training and Supervised Billing. $750/month*
Individual face to face clinical supervision rate: $150./session
Clinical Supervision
Individual sessions with Kathleen and the supervision team offer the opportunity to present challenging cases for clinical support, to focus on general counseling skills, counselor development, general counseling topics and concepts of relevance, practice management, documentation, ethics, or crisis management. This program includes assessment, collaborative goal setting, and evaluation.
Group Supervision rate: $150/hr (group is 2 hrs.) divided by the number of participants. We offer monthly general group supervision, and specialty supervision groups for gender affirming care, school-based practice, and couples/family counseling.
Administrative Supervision and Training Program
*included in the subscription service fee includes:
- the ability to provisionally work and bill insurance claims under the license and supervision of Kathleen Landry or a member of the supervision team, as per the regulations of the state of Vermont and Vt. Medicaid.
- Each clinician is given a sub-account of OpenGate’s Simple Practice account, this is a complete office management system which includes scheduling, client database, telehealth services , HIPPA secure electronic charts, and more. This account is linked to your Supervisor, and all of your charts and documentation can be signed off easily. Here you can also track your earnings and hours.
- We offer mentorship, education, training, and oversight of a complete curriculum of private practice management skills, including both clinical and administrative skills. Materials, including guides, checklists, and handbooks, and access to some groups and all classes and workshops throughout the year are all included in the cost.
- We provide some referrals, and a listing on our website directory, as well as caseload development strategies and support.
- Kathleen holds open office hours for administrative support on Wednesdays and Fridays between 8am and 4pm. For all calls under 15 minutes, there is no charge for this service.
- Clinicians now have access to our secure Clinician Hub, where they can access all of the resources and training materials included in the program, anytime, from anywhere, all organized carefully, as well as tools announcements, and more!
- NOT INCLUDED, but optional add-ons include AI Note Taker at a charge of $35./month (after 30 day free trial)
- Your first month of subscription fees are waived with a six month or more commitment!
Reimbursement:
- OpenGate pays the full amount of reimbursement by insurance companies to clinicians for all sessions, minus supervision fees and services, which are deducted from your bi-weekly paycheck. Payments are made to clinicians every other Friday via direct deposit.
- We accept BCBS, Medicaid, MVP, and bill out of network to Cigna, as well as direct payments via checks and credit/debit cards. (Interns can not accept BCBS until they graduate)

Kathleen’s Philosophical Framework for Supervision Practice
My supervision model is an integrated and flexible approach that combines several established theories and methods to ensure a comprehensive and supportive supervision experience. This model is deeply influenced by my years of experience in the field of adult education, as well as the educational principles of John Dewey and Maria Montessori. Their philosophies emphasize experiential learning, critical thinking, and individualized education.
I foster a mutual and reciprocal relationship with my supervisees through the Interactional Model, emphasizing the Supervisory Working Alliance. This involves setting collaborative goals and objectives while building a strong emotional bond of caring, trust, and respect.
At times, I employ a Patient-Centered approach, acting as an uninvolved counselor/advisor and focusing on the client’s needs rather than the supervisee or the supervisor-supervisee relationship. In other instances, I utilize a Supervisee-Centered approach, concentrating on the supervisee’s experience as a counselor.
My supervision also incorporates the Holistic Model, emphasizing trust, safety, positive regard, congruence, acceptance, and authenticity. This model utilizes a systems approach that highlights parallel processes and a strength-based perspective, viewing supervisees as colleagues and focusing on their strengths rather than solely on problems.
I integrate Competency-Based Supervision to ensure that supervisees develop the necessary skills and competencies required for effective practice. Additionally, I incorporate the Existential-Humanistic Therapy Supervision Model to support supervisees in exploring existential and humanistic aspects of their work, and the Cognitive-Behavioral Therapy Supervision Model to provide structured and evidence-based guidance in cognitive-behavioral techniques.
This eclectic and adaptive approach ensures that supervision is tailored to the unique needs and developmental stages of each supervisee, promoting their growth and professional development in a supportive and structured environment.
