Medicaid Enrollment Support

Medicaid Enrollment (Required for Pre-Licensed Clinicians)

As of January 1, 2026, all new pre-licensed clinicians who wish to bill Vermont Medicaid are required to be individually enrolled with Medicaid, either as an independent provider or as an individual provider within the OpenGate group.

This process is not optional, and it can take 30–90 days from start to finish. Because of that, Medicaid enrollment should be the very first step of your onboarding process at OpenGate. Please begin this process immediately upon joining—do not wait until you are close to seeing Medicaid clients.

What You’ll Need to Get Started

To complete your Medicaid enrollment, you will need the following:

  • A copy of your official graduate transcripts
  • An NPI number (individual)
  • Active malpractice insurance
  • signed supervision letter from me (with an inked signature)
  • Your signed attestation confirming that:
    • You have not yet completed your required hours
    • You are practicing under supervision

  1. How do I register with OPR (Office of Professional Regulation: here is the linkhttps://sos.vermont.gov/opr/
  2. Documentation requirements for application under licensure or graduation:
    1. PDF or word document showing the hours you have obtained
    2. Attestation between the supervisor and supervisee, must have the date, and signature of both
  3. Is it sufficient for graduate supervisees to have an unofficial transcript versus an official one? Must be a copy of a transcript or diploma.
  4. NPI is required for application: NPPES is where you apply for an NPI, please note non-licensed providers can obtain an NPI without a license if you have questions please outreach to NPPES directly. Link to NPPEShttps://nppes.cms.hhs.gov/login.

Hello,

As per the conversation in the webinar yesterday around the malpractice question, I have been informed that if the individual has their own malpractice insurance, this information should be added into the malpractice section of the enrollment application.

If they do not have their own malpractice insurance, they need to have their supervisor’s insurance information, or if the supervisee is not on a the insurance the supervisee must submit a note from the supervisor indicating that the supervisee can operate under their insurance.
